Services Provided by SugarSight<\/b><br \/>SugarSight provides a platform that facilitates the following services for you to engage with<br \/>patients:<br \/>\u2022 <b>Teleconsultation<\/b>: A secure, end-to-end encrypted video conferencing service for<br \/>consultations with patients.<br \/>\u2022 <b>Patient Management:<\/b> A dashboard to view and manage patient data, including<br \/>glucose readings, log entries, and uploaded medical reports, subject to patient consent.<br \/>\u2022 <b>E-Prescription &amp; Documentation:<\/b> Tools to generate digital prescriptions, medical<br \/>notes, and suggestions.<br \/>\u2022 <b>Scheduling:<\/b> An appointment booking system for patients to schedule consultations<br \/>based on your provided availability.<\/p><p><b>2. Dietician&#8217;s Responsibilities<br \/><\/b>As a Dieticianon the SugarSight platform, you agree to:<br \/>\u2022 Hold and maintain a valid medical license and registration with the relevant medical<br \/>council in India.<br \/>\u2022 Dietician to provide VTPL the medical license number, degree certifications and other<br \/>relevant professional qualifications.<br \/>\u2022 Provide accurate and up-to-date information regarding your qualifications,<br \/>specialization, and professional background.<br \/>\u2022 Adhere to all applicable laws, regulations, and ethical guidelines, including the<br \/><b>Telemedicine Practice Guidelines, 2020 <\/b>issued by the Government of India.<br \/>\u2022 Ensure the highest standards of professional conduct and exercise due diligence and<br \/>professional judgment in all consultations and medical advice provided.<br \/>\u2022 Maintain strict patient confidentiality and protect sensitive personal data in accordance<br \/>with the <b>Digital Personal Data Protection Act, 2023 (DPDP Act) and our Privacy<\/b><br \/>Policy.<br \/>\u2022 Not prescribe medications from the prohibited list as per the Telemedicine Practice<br \/>Guidelines<\/p><p><b>3. Financial Terms and Payment Agreement\u00a0<\/b><\/p><p>This section details the financial arrangements and payment structure for services rendered<br \/>through the SugarSight platform.<br \/>\u2022 <b>Appointment Fee:<\/b> The fee for each consultation is determined by you and is displayed<br \/>to the patient on the app. The range of the fee can be 100-1000 per appointment.<br \/>\u2022 <b>Revenue Share: <\/b>SugarSight will collect the full appointment fee from the patient on<br \/>your behalf. As a consideration for providing the platform, technology, and patient<br \/>base, SugarSight will retain a portion of this fee.<br \/>\u2022 <b>Payment to Dietician:<\/b> You shall receive 80% (eighty percent) of the gross appointment<br \/>fee. The remaining 20% (twenty percent) shall be the service fee retained by<br \/>SugarSight.<br \/>\u2022 <b>Deductions:<\/b> The following statutory deductions will be made from the gross<br \/>appointment fee before payment is disbursed to you:<br \/>o <b>Goods and Services Tax (GST): <\/b>SugarSight is responsible for remitting GST<br \/>to the government. We will deduct 18% GST on our 20% service fee.<br \/>o <b>Tax Deducted at Source (TDS):<\/b> We will deduct TDS as per the applicable<br \/>Indian Income Tax laws on the gross appointment fee. A TDS certificate will<br \/>be issued to you as per the legal requirements.<br \/>\u2022 <b>Payment Schedule:<\/b> Payouts will be processed on a monthly basis, covering all<br \/>completed consultations within the preceding monthly period. Patient Data and Privacy<\/b><\/p><p>You acknowledge that patient data is confidential and sensitive. You must handle all patient<br \/>information in accordance with our Privacy Policy. SugarSight will provide you with access to<br \/>patient data only with the explicit consent of the patient.<b><\/b><\/p><p><b>6. Disclaimer of Liability<\/b><\/p><p>SugarSight acts solely as a platform to connect you with patients. We are not a healthcare<br \/>provider, and we do not provide medical services. You are solely responsible for all medical<br \/>advice, prescriptions, and services you provide to patients through the app. SugarSight shall<br \/>not be held liable for any professional misconduct, negligence, or any claims arising from your<br \/>professional services.\u00a0<b><\/b><\/p><p>This agreement may be terminated by either party with a weeks written notice.
